New Patient Registration
We welcome new patients who live in Tilbury and Grays postcode areas of RM16, RM17 and RM18 only.
If you wish to register with us you can do so during normal opening hours.
Come to the surgery and ask to complete a registration form. You should bring your NHS Registration Card (which will be returned to you). You should also bring some form of photo ID such as passport, driving licence or photo card, and to confirm your home address either a recent bank statement, utilities, rent or council rates bill.
When you first register you must make an appointment for a general check-up with our practice nurse or healthcare assistant, who will also complete your registration.
Named accountable GP: You will be assigned to a doctor who will be your "named Accountable GP" as required by the NHS, however, you can choose to be seen by any clinician as long as they are available.
You will be added to the practice list, not an individual doctor. You can ask to be seen by any of the doctors in the practice although it will not always be possible to see the doctor of your choice on a particular day and time.
